4. Team Around the Family (TAF) meetings
The TAF meetings bring together the parents, adults, children, young people and practitioners. The assessment will be discussed and an action plan will be created which will address the identified needs. All key agencies and household members will all have actions to complete in order to support the family to achieve the planned outcomes.
The TAF will then meet at an agreed review date to evaluate progress and update the plan accordingly. The TAF will continue to meet approximately every 6 weeks until there are no outstanding actions on the plan.
