1. Change name or address of licence holder
You can apply to change the name and address of the licence holder on your premises licence by using the form below. If you need to make other changes to your licence e.g. DPS, licensable activities, operating hours or the layout of your premises then please see the relevant pages above.
Licensing: Licence holder change of details and request form
There is a fee of £10.50.
